SMP Aftercare: Complete Scalp Micropigmentation Healing Guide.
Proper SMP aftercare is part of the treatment process. Your scalp needs time to heal, the pigment needs time to settle, and the healed result helps determine how future sessions should be approached.
This practitioner-written guide explains the aftercare instructions I give clients at Michael Angelo SMP, including washing, Aquaphor, sweating, hats, sun exposure, shaving, swimming, healing and what to expect between SMP sessions.
How Long Does SMP Take To Heal?
At Michael Angelo SMP, I generally consider approximately two weeks an important healing window after scalp micropigmentation.
The most noticeable early effects, such as redness and the darker appearance of freshly placed pigment, usually begin changing much sooner. During healing, the pigment settles, the skin recovers and the treatment begins moving toward its true healed appearance.
Why SMP Aftercare Matters.
The way scalp micropigmentation looks immediately after treatment is not the same as the way it will look after healing.
Your treatment continues developing after you leave.
Fresh pigment generally looks stronger immediately after it is placed. Temporary redness can also change the way the treatment appears during the first stage of healing.
As the scalp recovers, the pigment begins to settle and soften. That healed result gives me information about color, density, retention, blending and how your individual skin responded.
That is one reason SMP should not be judged based only on how the first session looks immediately after treatment.

The first day is about protecting freshly treated skin while the initial healing process begins.
Expect the treatment to look stronger at first.
Fresh pigment normally appears darker immediately after treatment. Temporary redness can also make the result look stronger than it eventually will after healing.
Avoid unnecessarily touching, rubbing, scratching or picking at the treated scalp.
Keep anything touching the area clean, including your hands, pillowcase, towels and headwear when headwear is eventually reintroduced.
Clean Hands
Avoid unnecessarily touching the freshly treated scalp.
No Picking
Do not scratch, pick or rub the treatment area.
Keep It Protected
Follow the water and product instructions provided for your individual treatment.
Clean Pillowcase
Keep surfaces contacting the freshly treated scalp clean.
Avoid Heavy Sweat
Give freshly worked skin time before returning to intense activity.
Do Not Judge Yet
Fresh SMP is not the same as the healed final result.
What To Expect During Days 2–3.
During the first several days, the scalp begins transitioning away from the immediate post-treatment appearance.
Many clients notice a significant reduction after the first day, although everyone responds differently.
Do not assume the darker fresh appearance represents the healed final color.
Monitor the scalp because one small section may need additional moisture while another area remains comfortable.
Aquaphor & Dry Scalp Areas.
Aquaphor may be used as part of the SMP aftercare protocol I give clients. The goal is not to repeatedly cover the scalp in a heavy layer of ointment.
One mistake I sometimes see is a client applying aftercare cream once and assuming every section of the scalp will remain equally moisturized all day.
During healing, one small area can become dry again while another remains fine. Monitor your scalp. If a particular section becomes noticeably dry or flaky, lightly reapply to that area according to your instructions.
Washing Your Scalp After SMP.
Cleanliness is important during healing, but freshly treated skin should not be aggressively washed or scrubbed.
Follow the washing timeline your artist gives you.
Avoid aggressive rubbing, scratching or scrubbing during early healing.
Once water is introduced according to your aftercare instructions, use a gentle approach. A light cool-water rinse may be appropriate once the initial early healing period has passed depending on the treatment.
Avoid unnecessary shampoos, soaps, styling products or chemicals until they are appropriate for your healing stage.
Can You Wear A Hat After SMP?
In my aftercare approach, clients can generally return to a clean hat after the initial first 24 hours when appropriate for their treatment.
The important word is clean. Avoid putting a dirty, sweaty or heavily used hat directly against freshly treated skin.
Your individual instructions always take priority.
Before returning to clean headwear when approved by your artist.
Exercise & Sweating After SMP.
The gym itself is not necessarily the issue. Heavy sweating during early healing is what clients need to think about.
Give fresh SMP time before heavy exercise.
I generally recommend avoiding heavy sweating for approximately the first 48 hours.
Some people can perform light activity without sweating heavily. Other people sweat significantly even during moderate activity.
If your normal workout is going to leave your scalp covered in sweat, it is better to give the treatment more time.
Sun Exposure After SMP.
Avoid unnecessary prolonged direct sun exposure while freshly treated skin is healing.
Once the initial healing period has passed, you can live normally. Long-term, repeated intense UV exposure can contribute to gradual pigment fading.
Protecting a shaved scalp during prolonged or intense sun exposure can help maintain the appearance of your SMP over time.
Swimming, Saunas & Hot Tubs.
Avoid unnecessary soaking, prolonged moisture, heat and heavy sweating while the scalp is going through early healing.
Follow your artist’s timeline before returning to pools or extended water exposure.
Avoid sauna environments while the freshly treated scalp is still healing.
Hot water, soaking and prolonged moisture are not ideal during the initial healing period.
Shaving After SMP.
Do not rush to aggressively shave freshly treated skin. Give the scalp time to settle before introducing unnecessary friction.
Your shaving timeline depends on the work performed, your skin response and the method you normally use to maintain your hair.
Follow the specific timeline given by your SMP artist rather than assuming every client should shave at the same time.
Why SMP Looks Different As It Heals.
Healing is part of the visual transformation of scalp micropigmentation.
Fresh
Pigment looks stronger and temporary redness may be visible.
Healing
Redness decreases and the scalp begins recovering.
Settling
The visible pigment begins softening and lightening.
Evaluation
Your artist evaluates retention, color, density and blending.

The time between appointments allows me to see how your scalp actually responded before deciding what the treatment needs next.
Foundation
The first layer establishes direction, spacing and initial density.
Healing
The scalp heals and the fresh pigment settles and lightens.

I evaluate retention, healed color and individual skin response.
Layering
Additional density, texture and blending can be added.
Refinement
Smaller adjustments bring the treatment together.
Some clients can finish in two sessions. Others may benefit from an additional refinement session depending on the treatment, healing response, lifestyle and pigment retention.
There is no wrong number of sessions. The goal is to complete the treatment appropriately for the individual client.

Common SMP Aftercare Mistakes I See.
Ignoring Small Dry Areas
Some areas may dry again during the day. Monitor the scalp and lightly reapply aftercare product where instructed.
Sweating Too Much Too Soon
Gym clients sometimes return to heavy workouts before the scalp has had enough early healing time.
Judging Session One
The first layer is groundwork. SMP can still be darkened, blended, softened and refined.
You’re Not Going Through The Healing Process Alone.
Aftercare is not simply handing a client a sheet of instructions and sending them home.
